What is this for?

We want every person in Doncaster to live in the place they call home with the people and things that they love, in communities where they look out for one another, doing things that matter to them.

If you, or someone you know, needs some support to do these things, we can help.

What is it going to cost?

There is no cost in contacting us.  

What we need from you

If you are contacting us about yourself, we will need your name, contact details and your reason for contacting us.

If you are getting in touch with us about someone else, we will need your name and contact details as well as their name and contact details and your reason for contacting us.   

If you are contacting us about more than one person you will need to submit a contact form for each person. 

What you should expect from us

When you contact us, we’ll make sure you’re connected to the team who can best support you. This might be your local social work team, our Community Adult Learning Disability team, or our Occupational Therapy Team.

We’ll get in touch to listen and understand what’s important to you, and what you might need help with.  Then we’ll work with you, and anyone else you’d like us to involve, to make sure you have the support you need to live your life in the way you want to.

To help us support you, we ask for a wide range of information, some of which is personal and may be required by law. We do not collect more information than we need to and we will not keep it for longer than is necessary.
