What we need from you

To complete the form as fully as possible, giving as much detail as you can about your concern and any immediate actions taken to ensure that the adult at risk is safe. Individuals can report their concerns anonymously, but this may restrict the action we can take as we won't be able to ask you for more information. Referrals from partner organisations should still be made by signing in or registering for a My Doncaster account.

What you should expect from us

We aim to look at your concern within 1 working day and prioritise for action and allocation.
