Housing Benefit and Council Tax Benefit Forms

To claim Housing Benefit and Council Tax Benefit (including Second Adult Rebate), download the main application form below and any of the extra forms that apply to you. You will need to print the forms and complete them by hand before sending them to us.

Complete the Proof of Earnings form if you or your partner are working and you are unable to provide pay slips with your application.

If you are currently being paid Housing Benefit by cheque, you can ask us to make payments direct into your bank account.
