Special Educational Needs Forms

SEN1 Referral Form
This form should be used to refer a child for an assessment of special educational need. The form should be fully completed, signed and dated before submission to Special Educational Needs at the above address.

Appendix B Form
This should be used to provide educational advice towards a child's assessment of special educational needs. The form should be submitted to the Special Educational Needs Administrative Officer for your school.

Annual Review Document
This should be completed as the report of a child's Annual Review Meeting and should be sent to the Local Authority no later than ten days after the meeting or the end of that term, whichever is the earlier date. The form should be submitted to the Special Educational Needs Team.

The Future Needs Plan
This should be completed following review meetings for pupils in Year 9 and then updated at each meeting thereafter. The form should be submitted to the Special Educational Needs Administrative Officer for your school with the Annual Review Document.

Woodfield Primary School Additionally Resourced Centre for Children With Speech, Language and Communication Needs

This form should be completed if you have significant concerns about the extent to which strategies employed at School Action or School Action Plus have failed to enable a child to make expected levels of progress in the acquisition of speech and language skills. Children do not require a Statement of Special Educational Needs to attend an Additionally Resourced Centre. Please complete this form as fully as possible; you may if you wish make telephone contact with the teacher of the ARC at Woodfield Primary School, your school's Educational Psychologist, or SEN Officer prior to submitting this form to Woodfield Primary School. It is of course essential that prior to completing this form you have obtained permission from the child's parent to provide information about their child to staff at Woodfield Primary School.

In recent years there has been a decline in the number of referrals to Woodfield ARC. In part this is considered to be a consequence of school staff across across the Borough acquiring greater expertise in supporting children with speech and language difficulties and the development of high quality provision for children in their early years. The Management Group responsible for the ARC at Woodfield Primary School is keen to ensure that the service is provided to all children who have the potential to benefit. If you are in doubt please seek further guidance.
