About the Night Visiting Team

The Night Visiting Service is available to service users who are unable to be left unattended for the whole night.

The team consists of  Home Care Assistants who are supported by a designated manager working across the whole borough of Doncaster. Based at Barclay Court, our specialist team pride themselves on working closely with multi agencies, to provide a holistic approach to home care. The Night Care Team provides a service to adults on a long term basis depending on their individual needs.

The Night Care Team operates throughout the Doncaster Borough throughout the night, from 9.00pm to 6.00am, from Monday to Sunday including Bank Holidays.

The night care service will be available to all vulnerable adults with an assessed need, offering up to three visits nightly.

Objectives:

• Provide care during the night to service users with an assessed need. 

• Prevent hospital admission 

• Promote joint working with colleagues and other agencies

• Assist vulnerable people who are frail, terminally ill or at risk. Address their physical and mental well being and provide short or long term care whilst at home

• Assistance with prevention of pressure sores

• Monitoring and safety checks

• Monitoring of wellbeing 

• Assistance with toileting

• Assistance with medication

• Provide support to carers
